 Its a big discussion about if Egypt includes the Egypt of today or not because the people and culture is different then wayy back then. The land is the same but the country and land is not.
 But besides the Rambam also wrote in his letters that he dwells in land of sin (or something like that) when referring to himself when he was in Egypt. Also their could have been extreneous circumstances that lead to this. Don't forget he was in the run first from Spain.

 Present day Iraq. Today this thought and opinion does not apply, at the time it could have (Mahloket also Gemmarah Kiddushin towards the end) since the majority of the Torah world was in Bavel, some therefore held that since Jews were exiled to Bavel (Present day Iraq) and the vast majority of the Torah centers were there- since in Israel their were many wars and other problems, one should stay in Bavel for the Torah and religious life. Today almost no Jews in Iraq, and the greatest amount both in quality and quantity is in the land of Israel.

 I'm not sure perhaps Chaim can answer why he said this- perhaps a Middat Hassidut on his part, but I would think that if he would move out of NY to let's say Florida especially under the circumstances he is currently in (physically being prevented from fullfilling the Mitzwah) he would be allowed. Perhaps he does not want to move to a place where he will enjoy which might take his mind of yearning to go up to Israel.
 Also their are ideas and laws of not majing the Galut a permanent dwelling but something that is temporary (for example not exceeding a certin amount in building fences around one's property, I believe soo not to make extremly permanent dwellings outside the land).

 Usually almost everyone says that a Jew should plan to go up to Israel, but the better way is to do soo with the proper preparations. If one is not prepared enough they can move to Israel and then unfortunatly wind back in the exile because they simply were not ready to come. One should make plans, learn the language, have some savings and a plan as well (which is recommended by many) soo one will be stable and come to Israel with a permanent dwelling as opposed to going back and forth and G-D forbid giving up.
